About

We provide professional geriatric care management services to families who are caring for an elderly loved one. We conduct a comprehensive assessment to determine if the current care level is appropriate and if they are recieving the highest quality of care available. We then create an action plan giving the family guidance on how to attain the highest quality of life for their loved one. We also provide on-going care management for elderly individuals whos children live out of state. That care management includes advocacy and on-going consultation. All of our care managers are Certified Members of the National Association of Professional Geriatric Care Management and hold a Masters degree in a human services field.
