Franciscan Home Care and Hospice Care is a service of the Franciscan Family Care Center founded by the Franciscan Sisters of the Eucharist. The services offered by the agency include nursing and respite care; medication management and monitoring; home health aides and hospice care; and medical social services. It offers various rehabilitation programs, which includes physical, occupational and speech therapies. The additional services offered by the agency include homemaking, shopping assistance, medical supplies, housekeeping and meal preparation. It is licensed by the State of Connecticut and is approved by the Community Health Accreditation Program. Franciscan Home Care and Hospice Care is located in Meridian, Conn.
