Heritage Park Care Center, located in Carbondale, Colorado, is proud to offer a continuum-of-care campus for the community.  The following services are conveniently located on our campus: assisted living, post-acute skilled nursing, long- term care, secured dementia care and outpatient rehabilitation services. Rehabilitation is a strong focus at Heritage Park, our 3000 square foot inpatient and outpatient gym, features a therapy pool and state-of- the-art equipment.
